Elexicon is regulated by the Ontario Energy Board (OEB). The OEB requires that Elexicon summarize the terms of its contract with you in a document called the Conditions of Service, which may be amended from time to time. When you open an account with Elexicon, you agree to comply with its Conditions of Service. This includes paying all charges authorized by the OEB from the date on which service is connected and until you notify Elexicon to terminate the service. The details of all of the authorized charges are contained in Elexicon's approved Tariff of Rates and Charges. I/We confirm that the information provided is accurate and that I am/we are requesting service from Elexicon. I/We accept responsibility to be billed for the services provided as of the start of service date and agree to abide by the Conditions of Service.
